Incident Reports

Forms for Violation of Graduate Academic and Professional integrity incident report can be found on the North Georgia website. Each filed incident report will be submitted to the Associate Vice President for Academic Affairs, who then submits a copy of the incident report to the Graduate AIC. The Graduate Academic Integrity Council will then review all incident reports. Upon the filing of an incident report, the instructor or the student involved in the alleged violation may request that the Graduate Academic Integrity Council conduct a hearing and notify the Vice President for Academic Affairs. If neither the involved student or faculty member requests a hearing, but the council determines that the case justifies an investigation, the council may choose to conduct a formal hearing at its discretion. If the council chooses not to hold a hearing, then it will be recommended to the VPAA that the instructor-imposed sanction be upheld.
